Local Health Equity Action Team

The Local Health Equity Action Team (LHEAT) helps guide efforts to identify and address the root causes of health disparities in our community. Our team brings together voices from across sectors like health care, education, local government, nonprofits, and residents with lied experiences to work toward solutions that promote equity and opportunity for everyone.

THe LHEAT Lead helps plan meetings, providing updates from my roles as a Community Health Worker and BCBS Pathways grant coordinator, facilitating conversations, and making sure our work stays focused on equity. especially for those in our community who have historically been left out or underserved.

We review data, listen to community feedback, and build community partnerships. The goal is to make sure every person in Bourbon County has a fair shot at living a healthy, fulfilling life.

We have currently determined that the lack of an adequate safety net will be the focus of our work for the coming year and will work to address that issue.
